Payment

Insurance & billing

Our providers are in-network with AETNA, and we are out-of-network providers with the exception of AETNA.

Insurance plans may reimburse a percentage of our fees depending on your plan's out-of-network health benefits. We can provide invoices, upon request, with the information insurance companies generally require for reimbursement.

To make services more accessible, we have a limited number of sliding scale slots available. You can inquire about availability.

General

  • Intake appointment: 60 minutes, $560 / $410 (Dr. Aviva / Assoc)
  • Follow-up appointment / med management: 25 minutes, $260 / $190 (Dr. Aviva / Assoc)
  • Medication / counseling appointment: 60 minutes, $460 / $325 (Dr. Aviva / Assoc)

Recovery Boost Infusions

  • Recovery boost / infusion appointment: 60 minutes, Starting at $220
  • Replenish: $200 per infusion
  • Immunity Enhancer: $215 per infusion
  • Performance: $225 per infusion
  • Detox Ease: $215 per infusion
  • Beautify: $230 per infusion

Drug and Alcohol Monitoring

  • Remote breathalyzer monitoring: $400 monthly (Does not include cost of device)

Outpatient Detox

  • In-office detox: 5-7 days, $3,600
  • At-home detox: $1,500 per day (Up to 3 hours plus travel expenses; 4-day minimum)
  • At-home detox with live-in provider: $2,700 per day (Plus travel expenses; 4-day minimum)

Ketamine Assisted Psychotherapy

  • New patient / intake: $560 / $400 (Dr. Aviva / Assoc)
  • Preparation session: $450 / $315 (Dr. Aviva / Assoc)
  • Initial and subsequent KAP sessions: $1,450 / $950 / $850 (Dr. Aviva / Assoc)

GLP-1 Medications

  • Semaglutide / Tirzepatide: Starting at $250 monthly (Medication cost varies by medication and dose)

Cancellation Policy

We understand that things happen in life. Our cancellation policy requires 48 hours / 2 business days to cancel without a fee. There is a $50 charge for first cancellation less than 48 hours / 2 business days, and full session fees on subsequent cancellations. Please note we do not monitor our phones or email after hours and on weekends.

Phone calls exceeding 10 minutes that involve clinical discussion may be considered a follow-up consultation and billed at a rate of $75 per quarter hour.
